How Nathan Yoon Physiotherapy collects, stores, uses, and protects your personal information.

Nathan Yoon Physiotherapy (“we,” “us,” “our”) is committed to protecting your privacy and complying with the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs). This Privacy Policy explains how we handle personal information collected through this website (nathanyoonphysio.com.au), the booking system, and the clinic.

1. What information we collect

We collect personal information that is reasonably necessary to provide physiotherapy services, including:

  • Contact information: Your name, email address, phone number, and any details you provide via the website enquiry form or by emailing us.
  • Health information: Information about your symptoms, injuries, medical history, and treatment, collected during clinical assessment, treatment, and follow-up. Health information is treated as “sensitive information” under the APPs.
  • Booking and billing information: Information collected through Halaxy (our booking and clinical management platform), including your appointment history, billing details, and any private health fund or scheme details (DVA, WorkCover, NDIS, Medicare).
  • Website analytics data: If you accept cookies, we use Google Analytics 4 to understand how visitors use the website (pages viewed, referrer, country, device type). IP addresses are anonymised and we do not collect personal information through analytics. You can decline analytics cookies via the banner shown on your first visit.

2. How we use your information

We use your information to:

  • Provide physiotherapy assessment, treatment, and rehabilitation
  • Manage appointments, reminders, billing, and rebates
  • Respond to enquiries submitted via the website or email
  • Communicate with your treating GP, surgeon, or other practitioners with your consent
  • Bill third-party schemes (DVA, WorkCover, NDIS, Medicare, private health funds) where applicable
  • Comply with legal, regulatory, and professional obligations (including AHPRA and APA requirements)
  • Improve the website experience based on aggregate analytics

3. Disclosure of your information

We do not sell your personal information. We will only disclose it to:

  • Other healthcare providers — only with your consent (e.g. your GP, surgeon, or specialist as part of your care)
  • Third-party schemes — DVA, WorkCover Queensland, NDIS, Medicare, and your private health fund, as required to process rebates and claims
  • Service providers — including Halaxy (booking and clinical record platform), our website host (Netlify), and our email provider, all of whom are bound by their own privacy obligations
  • Legal authorities — where required by law, court order, or to protect health and safety

4. Storage and security

Clinical records are stored within Halaxy, an Australian healthcare-focused platform compliant with the Privacy Act and the My Health Records Act. Website enquiry submissions are received via email and stored in our email system. We take reasonable steps to protect personal information from misuse, loss, unauthorised access, modification, or disclosure.

Health records are retained for a minimum of seven (7) years from the date of last service, as required by Queensland health practitioner regulation. Adolescent and child records are retained until the patient reaches the age of 25.

5. Cookies and analytics

This website uses Google Analytics 4 to measure aggregate website usage (pages viewed, referrer, country, device type). Google Analytics sets cookies on your device. IP addresses are anonymised. You will be asked for consent to analytics cookies on your first visit; if you decline, no analytics cookies are set. To change your decision later, clear your browser cookies for this site and refresh the page.

No third-party advertising pixels or social-media tracking pixels are used on this site.

6. Accessing and correcting your information

You have the right to request access to the personal and health information we hold about you, and to request correction if it is inaccurate or out of date. To make a request, email info@nathanyoonphysio.com.au. We will respond within a reasonable timeframe, typically within 30 days.

For health records, requests may incur a reasonable administrative fee in line with Queensland Health regulations.

7. Complaints

If you believe your privacy has been breached, please contact us first at info@nathanyoonphysio.com.au so we can attempt to resolve the matter directly.

If you are not satisfied with our response, you can contact the Office of the Australian Information Commissioner (OAIC) at oaic.gov.au or call 1300 363 992.

8. Updates to this policy

We may update this Privacy Policy from time to time. The most current version will always be published on this page with the “Last updated” date at the top.
